Our High School uses this Behavior Reflections sheet. It makes students really think about their behavior, what causes it, and what happens because of it. The teacher, student, and parent have to sign this form, and return it to Admin.

A simple reflection sheet for junior students after an incident/issue has occurred. It designed for the student to discuss the problem with teacher and provide students with the opportunity to reflect upon what could be done better next time. Hope it is useful!

This reflection sheet flow chart provides students with an opportunity to self-reflect after a moment of frustration. It allows students to not only reflect on an incident or moment of weakness, but also think through where their frustration came, and what they can do differently next time. This behavior reflection is aligned to the ABC format: Antecedent of Behavior (what happened first) Description of Behavior (what was your reaction) Consequence for Behavior (what should be done next)

This is a sheet that I use with students who need to process their behaviors. It also serves as documentation for a student's perception so we can begin to understand their locus of control.
