Power of Attorney
I have my "Five Wishes" filled out and copies to my Primary Care Provider, Hospitals, and most importantly went through it with my husband who is my Medical Power Of Attorney (he speaks for me when I can't speak for myself). Have you filled out yours, or some type of living will. Please do so if you haven't.
Porter Storey, executive vice president of the American Academy of Hospice and Palliative Medicine in Glenview, Ill., says it isn't enough to establish a health-care power of attorney unless you have explained to your designee how you would like to be treated in case of incapacity. He also recommends writing a living will detailing your wishes. The living will and the power of attorney constitute what are called "advance directives"; some states consolidate these into a single form.
